Suncare Community Services is a not-for-profit organisation which, over the last 45 years, has operated in communities across south-east and central Queensland with empathy and compassion.

Its core purpose is to provide home care services so their clients can maintain the independence of living in their own homes. Their wide ranging services and programs are delivered with one guiding vision:

Making people’s lives better

The aged care system is difficult to navigate at the best of times. However, Suncare can draw on decades of experience to assist you in ensuring that your needs are met through specialised services that take into account your specific needs.

SUNCARE CORE PROGRAMS AND SERVICES

The Commonwealth Home Support Program offers entry-level support services that help senior Australians to maintain their ability to live independently. This program can help those in need of assistance with everyday activities such as grocery shopping, food preparation and household cleaning.

If you have more complex needs, a Home Care Package may be more appropriate. This option provides flexibility and allows you to take full control of the type of service provided, when and how it’s delivered and by whom.

Access to these programs is subject to a My Aged Care assessment. Suncare can help you navigate this process through their step-by-step guide.

Other programs Suncare provides include:

Short Term Restorative Care: A tailored packaged designed in collaboration with your GP. This eight week program aims to provide practical outcomes to improve health, wellbeing and ultimately a happier outlook on life.

Transition Care Program: Transition care programs are designed to maintain and improve physical or cognitive functioning during the first twelve weeks after returning home from hospital, enabling our customers to continue living independently in their homes, rather than entering residential care prematurely.

Maroochydore Community Centre: The Maroochydore Community Centre has been operating since 1987 and offers a multitude of events and activities as well as outings on the Suncare bus to the Coast’s natural beauty spots and attractions.

Active At Home: This is an exercise program delivered in the comfort of your own home that helps clients to regain mobility, regain strength and slow down the ageing process.

D Cafe: These dementia friendly events are held regularly at Rockhampton and Yeppoon and offer a chance for those living with dementia and their carers to socialise with others experiencing similar circumstances. Guest speakers are in attendance and can provide new information about carer support programs, they can also answer any questions which arise.

Murri Connections and Nandjimadji Art Groups: The Nandjimadji Art and Yarning Circle on the Sunshine Coast offers a creative and supportive environment for Aboriginal and Torres Strait Islander people and their carers who are living with a mental illness or disability. The group meet weekly at the Maroochydore Community Centre.

Through traditional and contemporary Aboriginal art, the artists tell their story, connect with their culture and maintain spiritual strength.

SUNCARE SUPPORT COORDINATION SERVICES

Suncare has developed a comprehensive Support Coordination Service through their extensive experience assisting those with complex needs. This positions Suncare as a uniquely qualified organisation to assist you with getting the most out of your NDIS plan.
